IEC Application - Please click on the link and print out the IEC application form. The application should be completed and signed with an original signature. All applications should be accompanied by financial documents and deposit(s). Please see our How to Apply link for more detailed information.

IEC Transfer Report - Please submit this form to your current school for completion. It should be sent with your other application documents and copies of your current I-20, I-94 and visa.

Student Health and History - Each student should complete this form in your home country. You are required to show dates for 2 Measles shots, 1 Mumps shot and 1 Rubella shot.

• Medical Insurance: Health and accident insurance is required of all IEC participants who come to the IEC with an F or J visa, and is provided through a SUNY-wide group policy. In addition, according to New York Public Health Law 2165, students must submit an immunization record that whows they have been innoculated for measles, mumps, and rubella. Students who do not comply with the above requirements will not be allowed to register for any courses.
